Damaged or Defective
Ouch! We know how disappointing it is to have a package arrive in damaged condition.
But don't worry we are here to make things right.
If the item was damaged in shipping, we likely have insurance to pay for the item and the cost to ship it. Even if we did not insure it, we will make sure you are taken care of! Really, we know you want to be treated fairly and want to do what's right for you.
Here is what we will need:

Step 2 Press the submit button.
Step 3 Send a photo of the item in it's original shipping container to:

With a photo of the damage you will likely not have to return the item. But please don't discard it until we have news from the insurance company. OK? Sometimes they want to inspect it.
By using this form you are certifying that this item was damaged in shipping. You might be asked to sign the following document by the insurance company as well:
Recipient / Buyer / Consignee Statement:
To be signed by the recipient (damaged packages) or intended recipient (lost packages)

Warning: Any fraudulent claims will make the shipper and/or consignee liable for any prosecution for mail fraud under federal crime code. The submission of a false, fictitious or fraudulent statement may result in imprisonment of up to 5 years and a fine of up to $10,000.00 (18 USC 1001). In addition, a civil penalty of up to $5,000.00, and an assessment of twice the amount falsely claimed may be imposed (31 USC 3802).
